The meaning of these words in application form..

a) Affiliation : Things you associated with. It means what groups and organizations you are connected with or a member of.

b) Designation : 1. The act of designating; a marking or pointing out.
                         2. Nomination or appointment.
                         3. A distinguishing name or title.
(In my opinion, no 3 gives clearer meaning for the title above.) ngee~~
